Subject – Application for the request of leave for my mother’s medical treatment
Sir,
With all due respect and honour, I would request a leave for tomorrow i.e.13th of September 2022 because of my mother’s medical treatment. My mother has been suffering from Arthritis and we have been making monthly visits to the hospital. Due to old age, the condition has gotten worse, especially with the cold weather.
This month, the visit to the doctor is scheduled for the 13th of September 2022. We currently have no family support at the moment, with my siblings working out of state and the maid who has gone to her hometown due to family problems. Concerning this, I would like to take leave tomorrow to take my mom for his medical treatment. Tomorrow’s treatment is extremely important as the doctor states because it will be the result day of whether or not my mother will be needing surgery.
I have requested Mr. Jaden to do the required task for tomorrow. I assure you that I will be back to resume my work by the 14th of September 2022 with no delay. I will put forth as much effort as I have lost during my leave and make up for the lost time.
I therefore kindly request you to grant me leave for which I shall remain forever grateful.
