Subject – Application for the request of leave for my mother’s medical checkup
Sir,
With all due respect and honour, I would request a leave for tomorrow i.e.13th of July 2022 because of my father’s medical checkup. My mother has been suffering from asthma and we have been making monthly visits to the hospital. My mother has also been recently caching cough and cold making her asthma condition even worse. This month, the visit to the doctor is scheduled for the 13th of September.
We currently have no family support at the moment, with my siblings working out of state. Concerning this, I would like to take leave tomorrow to take my mother for her medical check-up. I have requested Mr. Jaden to do the required task for tomorrow. I assure you that I will be back to resume my work by the 14th of July 2022. I will put forth as much effort as I have lost during my leave and make up for the lost time.
I therefore kindly request you to grant me leave for which I shall remain forever grateful.
