Subject – Application for the request of leave for my mother’s cancer treatment
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for two weeks I.e. 1st of January 2022 till the 14th of January 2022 because of my father’s cancer treatment. We were advised by the doctor of Royal Global hospital to go to Royal Global hospital in Chennai for his cancer treatment since that hospital is known for treating cancer at its best.
My siblings who are living out of state will be coming back to our home town only after two weeks. Thereby, we plan on taking turns. Concerning this, I would like to take two weeks off to take care of my mother for his cancer treatment. I have requested Mr Ken to do the required work for the one week that I will be missing on.I will put forth as much effort as I have lost during my leave and make up for the lost time. I will be back by the 15th of August 2022 with no delay.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
