Subject – Application leave for my father eye surgery
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for two weeks i.e. 14-10-2022 till 27-10-2022 because my father, age 86, has to undergo eye surgery. My father has been suffering from diabetes since he was about 40 years old. He has had health complications from time to time but because he has a very strict well maintained diet, he has remained healthy. However, diabetes can cause varied complications and with time it has affected his eyesight. He has diabetic retinopathy, this is a diabetes complication that affects the eye. It Is caused by damage to the blood vessels in the tissue at the back of the eye (retina). As scheduled by the doctor, we are to have the surgery for my father er on 15-10-2022 and would be under the doctor’s constant care for two weeks. Concerning this, I would like to take leave for two weeks to care for my father. I will be back to resume my work by 28-10-2022. I have assigned Miss Ella to take care of the necessary work for the next two weeks.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
