Subject – Application for the request of leave for my daughter’s surgery
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for two weeks i.e. 14-10-2022 till 27-10-2022 because my daughter, age 8, has to undergo surgery. My daughter has been suffering from stomach pain for the past few days so we visited a doctor and found that there have been stones in her stomach. As scheduled by the doctor, we are to have the surgery for my daughter on 15-10-2022 and would be under the doctor’s constant care for two weeks. Concerning this, I would like to take leave for two weeks to care for my daughter. I will be back to resume my work by 28-10-2022. I have assigned Miss Ella to take care of the necessary work for the next two weeks
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
