Subject – Application for the request of leave for my daughter’s treatment
Sir,
With all due respect and honour, I would like to take leave for tomorrow i.e. 7th of November 2022. This is because my daughter has been having trouble with her visuals. We have already contacted the eye doctor and have scheduled an appointment with the doctor for tomorrow. The doctor advised that we do the treatment at the earliest since my daughter is still young. That way, she will heal from having visual problems as she grows older or else if delayed she may start having more problems in the future. My husband is currently out of town and my parents are caught up with another work engagement. Thereby leaving none but me to care for her. Concerning this, I would like to take leave tomorrow to take my daughter for eye treatment. I will be back by the 8th of November 2022 to resume my work. I have requested Mr. James to take care of the necessary work for the two days that I am on leave.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
