Subject: Leave Application for Hand Surgery
Respected Sir
With humility, this is to bring to your kind notice that my hand surgery is due for 21st December 2022. I have met with an accident which has resulted in my right hand’s fracture. 15 days of strict rest to the hand after surgery has been prescribed to me along with various alterations in my food intake.
Thus I, Naina, Sales Head, hereby request you to grant me a leave from 20th December, 2022-4th January, 2023. I have already completed the assigned targets in advance so that the company doesn’t suffer any backlog. I assure you that I shall be present on 5th January, 2023 in the best of my health.
I will be really obliged to you for accepting my leave application and for allowing me to undergo this surgery without any work pressure.
