Subject: Leave Application for Eye Surgery
Respected Sir
With humility, this is to bring to your kind notice that my cataract surgery is due for 17th December 2022. I have been suffering from vision impairment of approximately 80%. It has now become difficult to see things even with bifocal spectacles. The subsequent check ups made the change in eye lenses a necessity.
The eye specialist has advised to undergo cataract surgery as this would simplify the matters to a great extent. 15 days of strictly indoor rest after surgery has been prescribed to me along with various alterations in my food intake.� Thus I, Naina, Sales Head, hereby request you to grant me a leave from 17th� December, 2022-4th January, 2023.
I have already completed the assigned targets in advance so that the company doesn’t suffer any backlog. I assure you that I shall be present on 5th January, 2023 in the best of my health.
I will be really obliged to you for accepting my leave application and for allowing me to undergo this surgery without any work pressure.
