Subject: Leave Application for Eye Check Up Appointment
Respected Madam,
With due respect and great pleasure, this is to bring to your kind attention that I, Maitreyi, student of class XII-A will not be able to come to school on 19th December, 2022 as I have to go for an eye check. My appointment has been fixed with the doctor. It’s been a few days that I can’t see things at a distance clearly.
It causes me a lot of trouble in classes along with subsequent headaches. The doctor has advised me to take a day off to rest and get my eyes checked. I will try to complete my homework when I come to school and will prepare the lessons for the upcoming examinations. Hence, I request you to grant me a leave on the above mentioned date. I assure you that I shall be joining back on 20th December, 2022.
I will be obliged to you for accepting my leave application.
