Subject: Leave Application for Ear Surgery
Respected Sir
With humility, this is to bring to your kind notice that my ear surgery is due for 19th December 2022. I have been suffering from severe ear pain for the last 5 days. Check up showed the accumulation of some waste in the inner ear. The waste now has to be removed through a surgery.
15 days of strictly indoor rest after surgery has been prescribed to me along with various alterations in my food intake. Thus I, Naina, Sales Head, hereby request you to grant me a leave from 19th December, 2022-4th January, 2023. I have already completed the assigned targets in advance so that the company doesn’t suffer any backlog. I assure you that I shall be present on 5th January, 2023 in the best of my health.
I will be really obliged to you for accepting my leave application and for allowing me to undergo this surgery without any work pressure.
