Subject: Leave Application for Husband’s Surgery
Respected Sir
With humility, this is to bring to your kind notice that my husband’s bypass surgery is due for 23rd December 2022. He has been suffering from heart blockage of approximately 80%. The subsequent check ups made the installation of stunts a necessity. But the doctor has advised to undergo bypass surgery as this would simplify the matters to a great extent.
15 days rest after surgery has been prescribed to him along with various alterations in his food intake. I have to be at home to take care of him as well as manage the household. He’s always been there for me and now it is my turn. Thus I, Ashwini, Marketing Head, hereby request you to grant me a leave from 23rd December, 2022- 7th January, 2023. I have already completed the assigned targets in advance so that the company doesn’t suffer any backlog. I assure you that I shall be present on 8th January, 2023.
I will be really obliged to you for accepting my leave application and allow me to undergo this phase without any work pressure.
