Subject: Leave for Cataract Surgery
Respected Sir
With humility, this is to bring to your kind notice that my cataract surgery is due for 3rd December 2022. I have been suffering from cataract for about 3 years and experience vision impairment of approximately 80%. It has now become difficult to see things even with bifocal spectacles. The subsequent check ups made the change in eye lenses a necessity. The eye specialist has advised to undergo cataract surgery as this would simplify the matters to a great extent. 15 days of strictly indoor rest after surgery has been prescribed to me along with various alterations in my food intake. Thus I, Naina, Sales Head, hereby request you to grant me a leave from 3rd December, 2022- 19th December, 2022. I have already completed the assigned targets in advance so that the company doesn’t suffer any backlog. I assure you that I shall be present on 20th December, 2022 in the best of my health.
I will be really obliged to you for accepting my leave application and allow me to undergo this surgery without any work pressure.
