Subject – Application for the request of leave for sick leave for class 4 student
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for three days i.e. 3rd of February 2022 till the 5th of February 2022 because I am suffering from a severe stomachache. I would like to take this leave so that I can go for a check-up with my father and heal properly. My mother is currently out of the station and so are my older siblings. I want to heal properly and get back to class.
My mother already consulted a doctor at Clean hospital and has asked to get me checked up immediately before any other problem arises. According to the doctor, he said that a lot of kids my age are suffering from stomach aches and he suspects a stone in the stomach or infections because all the kids that came to him from the start of last month had either one of these.
Therefore, he wants to conduct some blood and urine tests as soon as possible. I have already completed the assignments in advance and have submitted the project work to our class teacher, Sir Kenneth.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
