Subject: Leave Application for slip disc
Respected Sir,
Most humbly and respectfully, I want to bring your kind attention towards the fact that I have a slip disc.It happened during my trip last week and I went to the doctor for a check up. I am asked to take proper 0 and am currently under medication. I would like to request a leave for a week i.e. 1st of June 2023 till the 7th of June 2023.
I will be back by the 8th of June 2023 with no delay. I have requested Miss Jamie to be my replacement for a week and she happily accepted to do so. I have also completed my weekly task ahead of its time so that there is no backlog in my office work and in our annual goal.
Please accept my leave application and allow me to heal and be back to the office in good health.
