Subject – Application for leave for health issues
Sir,
With all due respect and honour, I would like to state that I have been suffering from a fever for the past 4 days and I would like to take myself for a check-up tomorrow at Aiden’s hospital. I have talked with the doctor on the phone and am advised to get myself checked before any complications.
I would like to request a leave for tomorrow i.e. 5th of July 2023 so that I can pay a visit to the doctor for my checkup. I’ll be back by the 18th of November 2022 with no delay. I will put forth as much effort as I have lost during my leave. I have requested Ma’am sang for the required task and she happily accepted to do so.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
