Subject: Leave for Cancer Treatment
Respected Sir
With humility, this is to bring to your kind notice that my cancer treatment will begin on 3rd December, 2022. I have been diagnosed with stage-1 cancer for a year. The subsequent check ups identified the presence of cancer cells in my mouth. The doctor has also advised me to initiate the treatment without any further delay. The treatment will include regular injection doses and kemo therapy. Thus I, Ashish, Marketing Head, hereby request you to grant me leave from 3rd December, 2022- 5th December, 2022. I will complete the assigned targets from home so that the company doesn’t suffer any backlogs due to me. I assure you that I shall be present on 6th December, 2022.
I will be really obliged to you for accepting my leave application and allowing me to prioritise my health.
