Subject – Application for the request of leave due to my mother’s health issues
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for 5 days i.e 21st of June 2022 till the 25th of June 2022 because of my mother’s health issue. My mother, age 80, has been suffering from a severe cold, cough and fever for the past week and has been under medical care.
My mother stays in my hometown which takes a day to reach. My siblings have been taking turns to look after my mother, however both my siblings have an urgent office meeting for which they won’t be able to be there for 5 days. I write this leave in respect to which I would like to take this time to take care of him.
I will be back on the 26th of June 2022 and resume my work. I will put forth as much effort as I have lost during my leave and make up for the lost time. I have requested Sir Manoj to take care of the necessary work during my absence.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
