Subject – Application for the request of leave for doctor’s appointment
Sir,
With all due respect and honour, I would like to state that I have been suffering from stomach pain and body ache lately. My taste buds have been feeling off as well. I contacted a doctor at Global Hospital and I am expected to have diabetes. However, I am not sure yet till I am tested. I was asked to make an appointment at its earliest to avoid other complications.
My appointment is scheduled on the 20th of May 2023 at 10 am. I will be back by the 21st of May 2023 to resume my work. I will be available on my email in case of any emergencies. I have requested Sir James to do the necessary work for the upcoming project. I have also submitted all works to our captain in charge, Ma’am Susan. I will put extra effort into the time lost.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
