Subject: Application for three days absent due to fever
Madam,
I am Mouni Roy, parent of Sandhya Roy, a student of class 9th B in your school. With due respect, I would like to inform you that my ward was absent from school for three days from October 22nd to October 24th.
She was suffering from high fever and was put on a drip. I requested her mathematics teacher to take a retest for her which she had missed on October 23rd and he has been kind enough to allow her for the same.
I request you to kindly consider these three days as the medical leave. I have attached her authentic medical prescriptions. I shall be immensely obliged.
