Subject – Application for the request of leave for my daughter
Sir,
With all due respect and honour, I Gina Luis, mother of Tiffany Luis would like to request a leave for three days i.e.13th of March 2022 till the 15th of March 2022 for my daughter Tiffany Luis would is in class 2 age 7 because she is suffering from a cough. With covid on the rise again, I do not want to take any risk.
I would like to take her for a check-up at Global kids hospital. My daughter Tiffany has very sensitive health, more towards why I do want to take any risk. I will bring her back to school by the 16th of March 2022 with no delay. I will be available via email for any urgent needs.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
