Subject – Application for the request of leave for mother’s checkup
Sir,
With all due respect and honour, I would request a leave for tomorrow i.e. 6th of February 2022 because of my mother’s checkup. My mother is a diabetic patient and therefore, we have to make frequent visits to the hospital to keep a check on her health. She has also been suffering from cold and cough and this has degraded her immune and appetite drastically. With her old age catching up, other complications keep rising.
We visit the doctor once every month and this time the appointment for his checkup is scheduled for the 6th of February 2022 at Barman Diabetes Specialities, Guwahati. I have requested Miss Marian to do the required task tomorrow in my absence and she has happily accepted to do so. I assure you that I will be back to resume my work by the 21st of February 2022. I will put forth as much as I have lost during my leave and make up for the time lost.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
