Subject – Application for the request of leave due to my wife’s treatment
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for 4 days i.e 13th of November 2022 till the 16th of November 2022 because of my wife’s treatment. She has been down with a cold and allergies for some time now. The next appointment to visit the doctor is scheduled for the 13th of November. We are told that she may be hospitalised for a few days as the doctors run some tests on her.
