Subject – Application for the request of leave due to jaundice
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for ten days i.e. 6th and the 15th of June 2023 because I am suffering from jaundice. Since last week, I have been having continuous fever, abdominal pain, chills, flu- like symptoms and with time there was change in my skin colour.
I went for a check up yesterday since I wasn’t getting better and had to undergo quite some tests and scanning. The results showed that I had pre hepatic jaundice. I am currently under medication and am requested to take bed rest for about a week. Concerning this, I would like to take leave for ten days to care for myself.
I will be back to resume my work by the 16th of June 2023 with no delay. I have requested Mr. Sammy to take care of the necessary work while I’m on leave. I will put forth as much effort as I have lost during my absence.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
