Sub: Application for miss punch
Sir,
With due respect and honour, I would like to state that I forgot to punch my attendance yesterday because I was late to catch the metro. My mother is severely ill and I got a call last minute that she was taken to the intensive unit care (ICU) at Royal Global Hospital. There is currently no family support at the moment.
My neighbours saw my mother passing out on the balcony and rushed her to the hospital. This news really scared me and I had to rush to the hospital. I am extremely sorry. I would not repeat this again. I would like to kindly request you to see to it.
I, therefore, kindly request you to grant me leave for which I shall remain ever grateful.
