Subject – Application for attendance
Sir,
With all due respect and honour, I would like to state that I had a very stomach problem after lunch yesterday i.e. 6th of July 2023, and was in the restroom, resulting in me missing my attendance. I was feeling quite a bit of a headache yesterday morning. I would like to state that I was there in the office yesterday but skipped my attendance due to the reason stated above. I would be so grateful if you could see to it.
More towards that, I also went to the infirmary and rested there for about an hour for rest since the pain made me not able to stand. I was given some medication by the nurse there as well and have signed my name there along with the allotted time spent there if needed for further observation.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
