Subject: Requesting for the issue of new Identity card
Sir,
I, Tom Hank, a student of B.A. Economics, Semester III, write this application to inform you that I have lost my Identity Card. I tried to search it wherever it was possible inside the campus and asked for it personally to people but I could not find it. I applied in the department office for a new card but as per the rule, it cannot be issued without your permission.
Therefore, I request you to kindly sign under the application letter and permit the issue of a new card as soon as possible as I am not able to enter the library and issue books for the coming internal exams.
