Respected Sir,
Most humbly and respectfully, I am writing this letter to inform you that I am suffering from an infection and as such, I need to take a day off from school. I have consulted a doctor and I have been advised to take a day off to recover and take the required medication. I have attached a copy of the medical certificate with this application.
I understand that this leave might affect my studies and I assure you that I will make up for the lost time by the end of the week. I humbly request you to grant me leave for one day and I look forward to your approval.
