Subject– Application for permission to change the name
Ma’am,
With all due respect and honor I, Z.R. Caleb A, of class 8, would like to state that I would like to change my name due to personal reasons. It has been so that ID prof cards are no longer accepting names with initials. This has caused me a great deal of difficulty, resulting in me not being able to open my bank account as well and therefore, I have to change my name on all my ID cards i.e. aadhar and pan cards. My new name will be Zayn Ryan Caleb
