I am writing this application to bring under your kind attention the fact that I have been suffering from a medical condition that now requires a therapy session of one week. The doctor who has been assigned my case practices in the city which is around 200 km away from my residence. Kindly take notice of my absence in the school for one week, so that I may get proper treatment. I shall join after full recovery.
