Respected Madam,
I am writing this application to get this to your kind notice that while cycling on the track, I had a little accident that has left me unable to attend the school. The doctor has advised x-ray scan and bedrest for about a week. Kindly take notice of my absence from the class for at least a week. I shall attend the school after full recovery.
