Good evening sir,
With all due respect and dignity, I would like to request a leave of absence for tomorrow due to my wife’s medical checkup. She has been taking medication for the previous two months and has a checkup scheduled for tomorrow, February 28th, 2023. I would like to take leave tomorrow, February 28th, 2023, to care for her and see to all necessary medical treatments. On March 1, 2023, I will return to work.

As a result, I respectfully urge that you give me my leave, for which I will be eternally thankful.
