Subject – Application for leave for mother’s death
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for three days i.e 5th of May to the 8th of May 2022 because of the death of my mother. My mother has been ill for a very long period now. With age, there were more health complications which rose.
She was admitted to the hospital last month and was admitted to the ICU last week. She lost consciousness the day before yesterday and was under ventilation. She took her last breath in Global royal hospital at 3 pm yesterday i.e. 3rd of May 2022. I would like to request this leave to travel back to my hometown to be there for her burial.
I will be back to school by the 9th of August 2022 with no delay. I have requested Miss Nana to take care of all necessary work during my absence. I will put forth as much effort as I have lost during my time absent.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
