Subject – Application for the request of leave for father’s heart attack
Sir,
With all due respect and honour, I would like to state that I would like to request a leave for a week i.e. 1st of June 2022 till the 7th of June 2022 because of my father’s heart attack. My father, age 88, suddenly had a heart attack during a program yesterday. My family urgently took him to the St.stephen hospital in my hometown. He is currently under the constant care of doctors. With age, we were informed that he needed to stay in the hospital for at least a few weeks to avoid other complications. I would like to take leave for the next week to go to my hometown and care for my father while my siblings and I take turns looking after him. I will put forth as much effort as I have lost during my leave and make up for the lost time. I have assigned my duties to Miss Agnes for the current project we are working on.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
