With due respect, this is to draw your attention to the fact that I, Anita Shahi, Marketing Head, have an appointment with a doctor for a blood test on 28th November, 2022. It’s a routine check up that I undergo every six months to ensure physical well being. The doctor examines my regular health issues and prescribes medicines or introduces some changes in habits in order to combat the disease. I hereby request you to grant me a leave on the above mentioned date. I will complete the targets assigned for the day from home. I assure you that I shall be present on 29th November, 2022.
I will be grateful to you for accepting my leave application as this would help me to conduct my routine check up on time.
