Subject – Application for the request of leave for my father’s surgery
Sir,
With all due respect and honour, I would like to request a leave of five days i.e. the 6th of May 2022 to the 10th of May 2022 because of my father’s surgery. My father has been having problems because of the pain in his stomach for some time now.
We consulted a doctor a few days back and the results came in, suggesting that my father will need surgery because of stones in his stomach, more towards that it has been said that it would be best to do it as soon as possible to avoid other complications, especially with his old age. I would like to take a leave for three days to care for him.
In our family, it is just me and mom to look after my father at the moment. Concerning this, I would like to kindly request you to allow me to take my leave so that I will be able to do the necessary. I will put forth as much effort as I have lost during my leave and make up for the lost time. I have already requested Mr Raj to do the necessary work in my place. I will be back by the 10th of May 2022 to resume my work.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
