Respected Madam,

I am a student of 9 B studying in your esteemed school. My mother had a very bad fall recently that injured her knee. She requires immediate knee replacement surgery.

The surgery has to be done in Mumbai and I will have to accompany her. This surgery is very crucial for her treatment. 

I humbly request you to grant me a leave for two weeks, from 1st October to 14th. I shall resume my classes on the 15th. Hoping for a positive response.
