Respected Sir,
With due respect and honour I would like to state that I, Sandra Keith bearing the roll no 43, from the science department would like to take leave for the next 5 days i,e 20th January to 25th January as I have to go for a medical check up in Guwahati . I have been suffering from severe muscle pain in my leg for the past few weeks, making me unable to walk or stand well.
This is my third check up and I have been recommended to visit the nerve specialist in Zion Hospital in Guwahati. I will start my journey on the 23rd of January and try to get all my medical check up done by the 25th and return to class at the earliest.
I will stay updated with the notes and lessons given in the class so that I don’t fall behind. I have submitted all my necessary assignments and have cleared all the class tests.
Therefore I kindly request you to grant me leave for which I shall remain ever grateful to you.
