Subject – Application for the request of leave for my daughter’s operation
Sir,
With all due respect and honor, I would request a leave for a week i.e. 12-02-22 till 27-02-22 because of my daughter’s operation. My daughter of eighteen years has been suffering from stomach ache for the past few weeks and just about last week, we consulted a doctor. We had to run a few tests and the results of the test came out to be a serious matter. She has stones in her stomach thereby, causing her tummy to ache. The doctor advised us to get surgery for my daughter as soon as possible. Concerning this, I would like to take leave for a week to care for my daughter. I have requested Mr. Smith to do the required task for the week. I assure you that I will be back to resume my work by the 28th of February 2022.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
