Subject: Leave Application for IVF Treatment
Respected Madam,
Most humbly, this is to bring to your kind notice that I, Smita, Technical Head, would not be able to come to the office from 27th December, 2022-1st January, 2023 because I have to go to the gynecologist regarding my IVF treatment. Previously, I’ve had two miscarriages and now I am unable to bear a child despite various efforts. Thus this time, the doctor has advised me to take proper rest after this treatment starts.
Hence the only resort now is this fertility treatment to see my dream of becoming a mother come true and bear an offspring. Thus, I hereby request you to grant me a leave on the dates mentioned above. I will complete my target assigned for the days when I will be on leave from home. I assure you that I will join the office soon in good health.
I will be obliged to you for accepting my leave application.
