Subject:- Leave for dentist appointment.
Respected Sir,
I am writing this application to inform you that from the past two days, I have continuous pain in my upper tooth due to which I feel very uneasy all the time. I have consulted the dentist and he has advised to go for x-ray scan so that he can find what’s wrong with my tooth. I would not be able to attend my duty tomorrow so please grant one day leave in my favour as I have booked an appointment at the dentist’s clinic for x-ray scan and other consultations to find a remedy for the pain. I would be highly obliged.
