Subject: Leave application for routine checkup
Respected Madam,
With due humility, this is to bring to your kind attention that I, Mehak, assistant, will not be able to come to work from 12th December 2023 because of my routine check-up. I was diagnosed with diabetes last month. I was advised by the doctor to start my routine check-up every month for the next 8 months. I am currently under medication under the doctor’s prescription.
I would like to request a leave for a day so that I can take care of myself and heal faster. I will try to complete my homework whenever possible and will prepare the lessons for the upcoming project. Hence, I request you to grant me leave for a day. I assure you that I shall be joining back soon in good health. I have requested Miss Ella to do the necessary work for the day of my absence.
I will be obliged to you for accepting my leave application and letting me take proper rest to recover properly.
