Subject – Application for the request leave for my medical reasons
Sir,
With all due respect and honour, I would like to state that I haven’t been feeling well since last week. I have been catching a cold and cough and it has not been getting better for a long time now.
I would like to take myself for a check-up before any complications arise and with the covid on the rise again I do not want to take any risk for myself, my family, and my colleagues. I have requested Miss Ella to take my place for all the necessary work for tomorrow. I will be back by the 28th of March 2022 to resume my work.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
