Subject – Application for the request of leave for child surgery
Sir,
With all due respect and honour, I would request a leave for a week i.e.14th of February 2022 till the 20th of February 2022 because of my child’s cataract surgery. My child has been having eye complications for years, and with age, it has gotten worse. We visited the doctor last week for an eye check-up at the global eye clinic, Chandigarh and we were advised by the doctor to go for cataract surgery as soon as possible for the best results.
My husband is currently out of the station, leaving just my babysitter and me to take care of her. With my parents’ complicated health issues I do not want them to strain themselves into taking care of my child alone thereby, I want to take this leave to take care of my child and be with him in the hospital.
I have requested Mr John to do the required task for the week. I assure you that I will be back to resume my work by the 21st of February 2022. I will put forth as much as I have lost during my leave and make up for the time lost.
I, therefore, kindly request you to grant me my leave for which I shall remain forever grateful.
